VUIT will assist Vanderbilt University faculty with exam, quiz, or survey scanning. This service is available to Vanderbilt University faculty only.

Charges or fees are dependent on services requested.
Obtaining OPSCAN Forms
Standard optical scan forms (200 items with 5 possible responses; 120 items with 10 possible answers) may be obtained at the VUIT Operations Center (Hill Center Room 140). If not completed in advance, you will need to complete a Scan Sheet Acquisition Request form.
Scanning OPSCAN Forms
Without special arrangement, we will scan forms obtained from us (200 items with 5 possible responses; 120 items with 10 possible answers). Scan results will by default be uploaded to Vanderbilt's Box Collaboration Tool , but results can be printed upon request. If not completed in advance, you will need to complete a Scan Sheet Scoring Request form upon drop-off of sheets to be scanned. All scan sheets must be organized where they are in the same order and face the same direction. A key sheet must be provided. A key is identified by bubbling in the Identification Number of a scan sheet with the following: 0000000001.
With special arrangement, we will scan forms designed for your specific needs. Design must be done by National Computer Systems (Pearson) in collaboration with the VUIT Operation Center.

All forms to be scanned should be brought to the VUIT Operations Center located on the Peabody Campus in the Hill Center, Room 140. This department is open 24/7/365. Once forms are scanned and reports generated, you may pick up your forms and reports from the same location.
